Dunno the specifics of sign-up rules, but the extended warranty has been debated. I'm getting it on my wife's Town & Country - it has dual digital temperature control, motorized doors, dvd player, - too many expensive doodads that if they break, would be expensive to fix. I didn't get it on my PT because it's simpler, and major component likeliest to fail is the auto tranny.
I have 75,000 miles on mine now, only problem (other than a pothole and a deer) was a slight miss cured by new (Crane) ignition wires.
No matter what you decide, if it's an auto, have the tranny flushed and filter changed and filled with ATF +4 or ATF + 5.
Quite sure to keep warranty good you have to follow maintenance schedule.
