Check this connector at the bottom of the fan, on the drivers side, it was melted on mine when my PT had a fan issue. If they reused the old harness, the contacts in this connector may be melted or show signs of arcing and not making a good connection. You can also apply a very small amount of dielectric grease to the connector prongs to help with making a good electrical connection.



How and where did they mount the relays, did they zip tie them somewhere?

This is how the relays should be mounted to the fan:



If they didn't mount them to the fan, that's not a proper installation. Since you had it done at a dealership, you are still under warranty and you should be able to take it to any other dealership to be corrected.

Did you look on your invoice and does it have a part number for the fan they installed, if so, what is it?

The fact that the relays are not mounted to the fan tells me that they got the bare fan from NAPA and reused the old relays and old wiring harness. If they had gotten the complete assembly the relays would already be mounted onto the fan housing.
