No codes at all for the headlight or the rare crank-but-no-start problem. The car runs fine just with delayed low-beam headlights as described in my initial post.

After some research and having 04 GT examined by a mechanic, I suspect that the ECM needs to be replaced. The car cranked but failed to start twice in January which fits the symptom of a bad ECM,...
