Welcome. From what I have read here on the forum, they do need your VIN number and milage. As far as voiding the warranty, only if S1 is proven to have cause the problem. Having a good dealer helps. And don't apologize for your English, its just as good as half of us here[]

Mileage needed so that when you get your new PCM your used GT will not show 0 miles...the PCM is where this information is recorded.

Good job with the English! I'll be learning French in a year or so to study in Europe. Im scared

Mileage is not recorded in the PCM, its recorded in the dashboard somewhere. That's why it's such a big deal to replace the speedometer, whereas a PCM swap is a simple DYI exercise.
I was explained that the mileage field in the order form is just a leftover from the form for another part and it's not relevant for Stage 1.

Don't know where DC stores the mileage. I have swapped PCMs several times on my '98 Neon R/T, from the stock, to the Mopar, to the AFX Race and never dropped a mile off the odometer. I would think the PCM in our GTs is pretty much the same.
