Default My cruiser shakes/jerks at idle and near 50mph

I have a 2001 pt cruiser with 106,600 miles on it. It started to shake/jerk awhile ago when it is near idle in park, drive, and reverse. It also jerks when the car seems like it needs to down shift at around 45-55 mph. I need help on solving this problem can anyone help?
