Is it possible to be 180 off when setting timing?

Default Is it possible to be 180 off when setting timing?

Just finished new head gasket, timing belt etc. on 02 cruiser, started it up & it acted like it was out of time. I had the marks perfectly lined up after installing belt. Motor idled rough & surged after you let off the throttle. I talked to a trusted mechanic & he said I must be 180 off. After tearing it down again, I'm uncertain how I could be 180 off when the cam sprockets say "up" & crank is at TDC. Help?

On the next revolution cams would simply be 'on'

Most likely, it's a tooth off.
