I've noticed the steering wheel "rocks" when you grab it on both sides. There's no push/pull looseness so it wouldn't make sense the wheel nut is loose. Feels real mechanical in nature - like worn splines. Car has never been in an accident.

Take your car to a reliable dealer now! You don't want to mess with the airbag, which you have to take out to tighten the steering wheel retaining nut. My daughter had this happen to her Toyota a couple years ago. There was a service bulletin for it. I thought there should have been a recall. It could be very dangerous.
