Do you have rear discs or drums? If drums, then you need to remove the brake drum and see if some of the brake hardware has failed. Could possibly be a spring is weak and letting a shoe shift when you hit it hard. If the drum looks okay, get some new shoes AND all new hardware. A hardware kit doesn't cost that much and neither do shoes. Replace BOTH sides at the same time. Don't half-ass it like some folks do. You need to also look at the wheel cylinder and make sure it's not leaking. If so, replace BOTH side wheel cylinders. If ones weak, then the other isn't far behind. Oh, and welcome to the forum. Go to the new member header and introduce yourself and tell us about your PT. Thanks

Welcome to the forum

What you describe I recommend new brake pads or shoes and also the new rotors. Also a basic brake fluid replacement is in order because of the year.
While you are at the rear doing the brakes carefully examine all the other brake parts.
